Please take a few minutes to complete the below evaluation sheet.
It is intended to assist our Association in training and upgrading the communication skills of our members.
Using the drop down boxes select your appraisal in regards to the area involved.
Date of Contest and Team Involved:
The crew was prompt entering the field, properly uniformed and neatly groomed.
The pre-game was cordial and conducted to your satisfaction with questions/concerns addressed.
The crew was consistently in proper positioned and hustled during the entire game.
Penalties were reported to the bench area giving type of foul and player number.
All penalties were properly assessed according to GHSA rules interpretation.
The crew was consistent and unbiased in the judgement calls during the game.
Your wing officials were firm, fair and professional in controlling the sideline.
The officials answered and addressed questions or concerns brought to their attention.
